D'Vida Wellness Clinic ("we," "us," or "our") operates this website at https://www.dvida.clinic. This Privacy Policy explains what information we collect through the website, how we use it, and the choices you have.

1. This Policy Covers the Website — Not Your Medical Record

This is an important distinction, so we want to be clear about it up front.

• This Privacy Policy covers information collected through this website — things like our contact form, appointment requests, newsletter sign-ups, and website analytics.
• Our Notice of Privacy Practices is a separate document that covers your protected health information (PHI) as a patient of the clinic — your chart, your treatment records, and your billing information. That notice is governed by the federal Health Insurance Portability and Accountability Act (HIPAA) and applicable Oregon law, and we provide it to you when you become a patient.

2. Please Do Not Send Health Details Through This Website

Our website contact form and general email are not secure channels and are not intended for protected health information. Please do not use them to send symptoms, diagnoses, medical history, treatment details, insurance ID numbers, or any other sensitive health information.

If you need to discuss your care, call us at 503.626.2166 or bring it to your appointment. If you send us health information through the website anyway, we will handle it as carefully as we can, but we cannot guarantee the security of information transmitted that way.

3.1 Information You Give Us

• Contact form: your name, email address, phone number, preferred language, and whatever you choose to write in the message field.
• Appointment requests: if you book online, you do so through Jane, our third-party scheduling provider. Information you enter there is collected by Jane under its own privacy policy and shared with us as your provider.
• Newsletter and updates: your email address and your communication preferences, if you subscribe.

• Device and connection data: IP address, browser type and version, operating system, and device type.
• Usage data: pages viewed, links clicked, referring site, and time spent on the site.

We provide care to patients of all ages, including children, and we welcome pediatric patients at the clinic. This website, however, is intended for adults. We do not knowingly collect personal information online from children under 13 without verifiable parental consent. A parent or legal guardian should submit any online request on a minor's behalf. If you believe a child has provided information through this site, please contact us and we will delete it.

10. How Long We Keep Information

We keep website inquiries and subscription records only as long as needed for the purpose they were collected, and then delete them on a routine schedule. (CLINIC-CONFIRM: set the actual retention window for contact-form submissions.)

Patient medical and billing records are kept for the periods required by Oregon and federal law, which are longer than website retention and are addressed in our Notice of Privacy Practices.

11. Security

We use administrative, technical, and physical safeguards intended to protect information from unauthorized access, including encryption of data in transit. No website or method of electronic transmission is completely secure, however, and we cannot guarantee absolute security. This is also why we ask you not to send health details through the website — see Section 2.

Our site links to third-party sites, including our scheduling platform and mapping services. Once you leave our site, this policy no longer applies.
